By completing this form, you’re helping us make contact with the victim as safely and quickly as possible. It is important to provide as much information as possible as this saves the victim from being asked the same questions multiple times and helps us to understand more about their needs and circumstances.
We can only accept referrals for victims who are aware that the referral has been made and have agreed to be contacted.

- We can not share information disclosed to us without the consent of the victim or the necessary legal sharing authorisation.
